Program Overview:
Members of the North Florida Health Corps (NFHC) serve full time with community-based organizations which provide services for the underserved in the North Florida area. Members are given the opportunity to gain invaluable hands-on experience and an inside look at the health care system while providing assistance to individuals who might otherwise not receive it. Members provide a variety of services such as health education, health screenings, and patient assessments. Members may also assist with community health workshops and education at community events and health fairs. The NFHC partners with community organizations to improve the quality of life for individuals in the North Florida area including homeless men, women and children by improving access to primary health and dental care, case management services, and linkage to other services such as insurance. In addition to being placed at a host site, members participate in outside volunteer activities since the NFHC is not just about going to a host site but it’s about getting involved in the community. Members participate in a group service project which is done monthly; a member meeting is also held monthly where members receive updates on the program, activities, each member's site, etc.

Members serve full time (~38 hrs per week) with their community host site and attend Corps trainings, meetings, and group service projects/events. The term of service is 46 weeks; members receive a stipend of $11,400 paid bi-weekly, health insurance and an Education Award of $4,725 upon successful completion of service.

JaxHATS mission is to ensure that youth and young adults ages 16 to 26 years of age are able to transition to and access the adult health care system, receive education on resources that can improve their quality of life, and be provided appropriate vocational rehabilitation training and education so as to meet their career goals.
The member will assist patients in online applications for Medicaid and will accompany patients who reside in Duval County to the Shands Evaluation Program to provide support in the completion of documentation needed to apply for that health care assistance program. The member may also work with patients and indigent drug plans of pharmaceutical companies to help patients to receive necessary medications.

The member will co-teach one-on-one and group diabetes education; safe sex/birth control education; and transition readiness assessment. In addition, the member will assist patients in accessing various websites related to smoking cessation, nutrition and exercise, transition readiness, violence prevention.
The member will participate in ongoing effort to immunize adolescent patients of JaxHATS. In addition, the member will spearhead the annual effort to identify at-risk patients in need of influenza vaccination due to their serious health condition, and will inform and schedule appointments as needed for them.

The member will distribute outreach materials, to consumers, schools, areas physicians, and community organizations. The member will gather health information, in print and electronic format and will distribute it to patients and caregivers under the guidance of the program manager.
